HOW TO ORDER
- Download our Script Form (click here to download) and ask your optometrist to complete
- Please check Page 2 of the downloaded Script Form or click here to see frame types available for your patient
- Please ensure you have filled in EVERYTHING on the Script Form
- Please ensure the optometrists fills in EVERYTHING and SIGNS the Script Form
- Please ensure you or the optometrist has indicated the patient’s choice of FRAME TYPE on the Script Form
- Scan or photograph the script form or script from the optometrist
- Please ensure that the Script is legible and not bigger than 6MB
- PLEASE NOTE ONLY ONE SCRIPT PER ORDER!!
- Fill out the online form below in full
- Please make sure everything is correct!
- Click on the UPLOAD button to attach your scanned Script
- Follow the instructions to upload the Script
- Click on the SEND SCRIPT button to send us your order
- Check your email entered for a confirmation email from us.
